Per local, statewide or nationally determined protocol and physician peer-reviewed recommendations or in compliance with those initiatives or regulations, the health coach will coach individuals adopt healthy behaviors, and will conduct outreach for medical personnel and Renown Health departments to implement programs in the practice that promote, maintain, and improve individual and community health. May also provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs, and may provide first line medical services. In addition, this position may collect data to help identify health needs. Nature and Scope:

Identify the unique health care needs of individuals in targeted cohorts.
Help customers in utilizing resources, including scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
Advise customers or cohorts on issues related to diagnostic/preventive screenings, such as breast cancer screening, pap smears, colorectal, or diabetes screenings. Also advising customers or cohorts on issues related to improving general health in regards to nutrition, physical activity and/or tobacco cessation. Also advising on issues related to mental health, substance abuse and emergency preparedness.
Chart review and assessment of care gaps or opportunities, and effective communication of those care gaps to customers and/or care team members, including the provider.
Advise, plan and set goals with customers or cohorts on disease management and issues related to self-care.
Contact customers in person, by phone or in writing or follow-up.
Be able to assist customers in creating a healthy vision and goals for their health & wellness program.
Participate in weekly meetings and monthly in-service trainings.
Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change.
Appropriately adapt telephonic coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each customer served.
Demonstrate established customer service behaviors and standards.
Challenges to this position are delivery of patient care complicated by new techniques, paperwork, changing department and hospital policies, increased information available on each patient, and fluctuations in staffing.
The incumbent uses professionalism and diplomacy with interacting with patients of all ages, their families, physicians, physician office staff and other health care providers in the accurate collecting of demographic, clinical and financial information in person or via telephone interviews.
This position will keep abreast of all standards to assure compliance with federal, state and local regulatory agencies and ensure compliance with federal, state and local regulatory agencies including third party reimbursement requirements.

Minimum Qualifications: Requirements – Required and/or Preferred Education: Must have working-level knowledge of the English language, including reading, writing and speaking English. Mathematical skills required. Experience: One (1) year of medical back office experience, or the completion of a medical assistant externship. Multi-physician office practice experience highly preferred. Current State of Nevada license for Office Laboratory Assistant, or an application must be submitted within 90 days of employment.
